Support Coordination – Coordination of Supports
This support will assist you to build the skills you need to understand, implement and use your plan. A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.

City to Country Care will work with you and your support network to assist you in exploring your options, educating you on how to navigate the NDIS and how to effectively use your funding by:
Implementing your Plan
We will get to understand who you are, what is important to you, and what you need. We will explain your NDIS plan and the support budgets you have been given, we will assist with engaging new services, arranging required assessments, and organising service agreements; a Support Coordinator is your go to person for all things relating to your NDIS Plan.
Monitoring and Reporting
It is our role to make sure you are happy with the services you are receiving and if not to look at solutions or alternatives to resolve this. Throughout the duration of your plan we will observe this with you and if required, make necessary changes to services.
We are responsible for reporting progress of your plan on a regular basis to the NDIS. We will complete all reports for the annual review, or if there is a change of circumstances, we will complete all necessary paperwork and reports for this as well.
Reviewing Your Plan
If you find during your plan that your needs have changed and your plan is not now meeting your requirements, we will assist you to complete documentation to lodge a change of circumstances form, collect and put together appropriate documentation and submit to the NDIS for consideration.
Prior to your plan ending we will meet with you and you can tell us if your goals are still relevant to your needs. We will contact your support providers to gather any reports or supporting documentation that will assist in the review process, to inform the NDIS of your support needs for the next 12 months.
